Carnival company fined after worker dies in NC

GREENSBORO, NC (AP) — A Georgia company faces a $27,000 fine
for safety violations after a carnival worker was killed in North
Carolina while dismantling a Ferris wheel in May.

The News & Record of Greensboro reported Tuesday
that the North Carolina Labor Department cited Event Coordinators Inc.
of Augusta, Ga., for 12 serious violations related to the
accident in Greensboro. The company has 15 days to appeal.

Investigators said 42-year-old Michael Douglas Cook, of
Columbus, Ga., was killed in the early-morning fall at the end of a
six-day carnival.

State worker safety investigators said they found a fitting
failed on a wire cable being used to take apart the wheel. That
allowed part of the wheel to fall and hit a 40-foot-high walkway
and an elevated basket holding a second worker.
